This is the typical situation in which an established group tries to disqualify rivals through unscrupulous means, as described by Drs. Waite and Reid.

You cost too much. You have to go. 

Another way to disqualify a rival is to accuse him of costing too much, which could occur just because he was treating sicker patients with intensive care, etc. The doctor under attack then faces a moral dilemma: Job security vs. professional standards.  This technique is offered by HMO's, industrial payors who want to pay the shareholders a dividend rather than pay for patients' care.
